It has been demonstrated that death certificates do not accurately record the actual cause of death in up to one-fourth of cases, as determined from subsequent autopsy findings. The purpose of this study was to explore the use of natural language autopsy data bases as an automated quality assurance mechanism. We translated the account of the major process leading to death, or the primary diagnosis, from all 45,564 narrative autopsy reports obtained at The Johns Hopkins Hospital between May 28, 1889, (...) and June 30, 1987, into the hierarchical system of Medical Subject Heading (MeSH) titles. We obtained a total of 125,772 MeSH title translations, 1,563 of them distinct (average 2.8 per case), ranging in frequency from 6,029 occurrences of LUNG to 1 occurrence apiece of 357 MeSH titles. The natural-language-to-MeSH translations showed expected trends over the past century: fewer infectious diseases; more cardiovascular and neoplastic disease among adults; and more respiratory diseases and congenital malformations in the pediatric age group. The greater availability of autopsy documents in electronic form should increase the value of this resource for quality assurance. (shrink)

In the past, hypothesis testing in medicine has employed the paradigm of the repeatable experiment. In statistical hypothesis testing, an unbiased sample is drawn from a larger source population, and a calculated statistic is compared to a preassigned critical region, on the assumption that the comparison could be repeated an indefinite number of times. However, repeated experiments often cannot be performed on human beings, due to ethical or economic constraints. We describe a new paradigm for hypothesis testing which uses only (...) rearrangements of data present within the observed data set. The token swap test, based on this new paradigm, is applied to three data sets from cardiovascular pathology, and computational experiments suggest that the token swap test satisfies the Neyman Pearson condition. Robert May shows that the South's expansionists had no more success than when they tried to extend slavery westward. As one after another of their plots failed, southern imperialists lost hope that their labor system might survive in the Union. Blaming northern Democrats and antislavery Republicans alike for their disappointed dreams, alienated southerners embraced secession as an alternative means to achieving the tropical slave empire that they craved. Had war not erupted at Fort Sumter, Confederates might have attempted to conquer the Caribbean basin. May's book serves as an important reminder that foreign policy cannot be divorced from the writing of American history, even in regard to seemingly domestic matters like the causes of the Civil War. Contending that America's Manifest Destiny became "sectionalized" in the 1850s, he explains why southerners considered Caribbean expansion so important and shows how southerners used their clout in Washington to initiate diplomatic schemes like the notorious Ostend Manifesto and presidential attempts to buy the slaveholding island of Cuba from Spain. He also describes southern filibustering plots against Latin American domains, such as the aborted designs on Mexico of the colorful Knights of the Golden Circle and the actual invasions of Central America by native Tennessean William Walker. Walker struck a major blow for the expansion of slavery when he legalized it during his occupation of Nicaragua. Most important, May relates how Caribbean plots affected American public opinion and ignited sectional friction in congressional debates. May argues that President-elect Abraham Lincoln might have saved the Union in the winter of 1860-61, had he agreed to last minute concessions facilitating slavery's future expansion towards the tropics. May's fascinating and often surprising account internationalized the causes of the Civil War. It should be read by anyone who wishes to understand the complex reasons why Americans came to blows with each other in 1861. I suspect that my idea of what it would be like to take a course given by William Barrett is fairly accurate. The flyleaf of his new book reports that Barrett, now Distinguished Professor of Philosophy at Pace University, received the Great Teacher Award at New York University. That note and a reading of Barrett's books, the classic Irrational Man, The Illusion of Technique, The Truants, and especially Death of the Soul, lead me to suspect that in the classroom Barrett (...) is as marvelous a communicator as he is on the written page. The concept of person is a primary interest of the contemporary intellectual world. Modern literature, films, theater, theology and philosophy focus their attention increasingly on the meaning of person. The current interests of philosophers can activate and direct their reading of the history of philosophy. The rereading of the history of philosophy with a new interest can lead to new insights and discoveries. Through these insights and discoveries, philosophies of the past come to life in the present and influence the (...) future. One of the tasks of contemporary philosophers is to read and examine philosophies of the past under the light of the current interest in person so that errors of previous philosophers might be avoided and insights of previous philosophers might be assimilated. The rereading, examination and exposition of past philosophers is and must be a community project. Both the vastness of philosophical literature and importance of dialogue, if progress is to be made, necessitate that the project be undertaken by many. This exposition of William Torrey Harris’ philosophy is offered as part of the project. Though Harris’ philosophy has been identified as a personalism, it has not received adequate study as a personalism nor has Harris’ concept of person been sufficiently examined. Robustness has long been recognized as an important parameter for evaluating game-theoretic results, but talk of ‘robustness’ generally remains vague. What we offer here is a graphic measure for a particular kind of robustness (‘matrix robustness’), using a three-dimensional display of the universe of 2 × 2 game theory. In such a measure specific games appear as specific volumes (Prisoner’s Dilemma, Stag Hunt, etc.), allowing a graphic image of the extent of particular game-theoretic effects in terms of those games. PurposeGiven the seeming lack of research on the influence of stakeholder activities on organisations such as political parties in the online environment, the purpose of this paper is to empirically investigate how political parties use their web sites to serve and manage their relationships with stakeholders.Design/methodology/approachThis is a qualitative paper, in which a cross‐national comparative analysis has been conducted on four illustrative cases. Personal interviews and web site observations were used as the main data collection methods. Three concurrent flows of (...) activities, i.e. data reduction, data display, and conclusion drawing, have been applied in the data analysis.FindingsThe paper indicate that the stakeholders served by political parties on the web sites studied can be grouped into two categories: internal and external. This paper also revealed that another very important internal stakeholder group overlooked by previous studies is that of core party members.Originality/valueThe paper provides insights and new interesting empirical material which may help to strengthen the theoretical understanding stakeholder management in cyber politics. Late in 1990, the Center for the Study of Ethics in the Professions at Illinois Institute of Technology (lIT) received a grant of more than $200,000 from the National Science Foundation to try a campus-wide approach to integrating professional ethics into its technical curriculum.! Enough has now been accomplished to draw some tentative conclusions. I am the grant's principal investigator. In this paper, I shall describe what we at lIT did, what we learned, and what others, especially philosophers, can learn (...) from us. 1 Evidence-based policy requires researchers to provide the answers to ecological questions that are of interest to policy makers. To find out what those questions are in the UK, representatives from 28 organizations involved in policy, together with scientists from 10 academic institutions, were asked to generate a list of questions from their organizations. 2 During a 2-day workshop the initial list of 1003 questions generated from consulting at least 654 policy makers and academics was used as a basis for (...) generating a short list of 100 questions of significant policy relevance. Short-listing was decided on the basis of the preferences of the representatives from the policy-led organizations. 3 The areas covered included most major issues of environmental concern in the UK, including agriculture, marine fisheries, climate change, ecosystem function and land management. 4 The most striking outcome was the preference for general questions rather than narrow ones. The reason is that policy is driven by broad issues rather than specific ones. In contrast, scientists are frequently best equipped to answer specific questions. This means that it may be necessary to extract the underpinning specific question before researchers can proceed. 5 Synthesis and applications. Greater communication between policy makers and scientists is required in order to ensure that applied ecologists are dealing with issues in a way that can feed into policy. It is particularly important that applied ecologists emphasize the generic value of their work wherever possible. (shrink)

About one third of patients with epilepsy have seizures refractory to the medical treatment. Electrical stimulation mapping is the gold standard for the identification of “eloquent” areas prior to resection of epileptogenic tissue. However, it is time-consuming and may cause undesired side effects. Broadband gamma activity recorded with extraoperative electrocorticography during cognitive tasks may be an alternative to ESM but until now has not proven of definitive clinical value. Considering their role in cognition, the alpha and beta bands could further (...) improve the identification of eloquent cortex. We compared gamma, alpha and beta activity, and their combinations for the identification of eloquent cortical areas defined by ESM. Ten patients with intractable focal epilepsy participated in a delayed-match-to-sample task, where syllable sounds were compared to visually presented letters. We used a generalized linear model approach to find the optimal weighting of each band for predicting ESM-defined categories and estimated the diagnostic ability by calculating the area under the receiver operating characteristic curve. Gamma activity increased more in eloquent than in non-eloquent areas, whereas alpha and beta power decreased more in eloquent areas. Diagnostic ability of each band was close to 0.7 for all bands but depended on multiple factors including the time period of the cognitive task, the location of the electrodes and the patient’s degree of attention to the stimulus. We show that diagnostic ability can be increased by 3–5% by combining gamma and alpha and by 7.5–11% when gamma and beta were combined. We then show how ECoG power modulation from cognitive testing can be used to map the probability of eloquence in individual patients and how this probability map can be used in clinical settings to optimize ESM planning. We conclude that the combination of gamma and beta power modulation during cognitive testing can contribute to the identification of eloquent areas prior to ESM in patients with refractory focal epilepsy.
